In life, a support system can help you through the difficult years of getting older or offer assistance with the disabilities that some people face. Interfaith’s Case Management Unit gives you that support by providing care management and consulting services to participants age 60 or older enrolled in the My Choice Family Care program (formerly the Milwaukee County Family Care program). Our case managers and registered nurses hold a wealth of case management and family care program experience to help you reach your optimum level of wellness and functionality.

It’s a team effort, and you are an important part of the decision-making process regarding outcomes related to medical, financial, environmental and social issues.

  • Identify appropriate providers and resources
  • Coordinate necessary services on your behalf
